To participate in the study as a patient, you must:
- Have a relapsing form of MS.
- This includes Relapsing MS (also called relapsing remitting MS or RRMS), clinically isolated syndrome (CIS), and active secondary progressive MS, which means SPMS with relapses.
- Have a scheduled appointment within the next 12 weeks with your healthcare provider to manage your MS .
- Be 18 years old or older.
- Live in the USA.
- Receive your MS care in the USA.
- Have access to the internet and a valid email address.
- Be able to read English.
- Agree to participate in the study.
